How to fill in an accident statement

📝 10 tips to fill out the form correctly:

  1. Pre-fill your info
    Complete your personal details (boxes 6–8) in advance to save time later.

  2. Be clear and legible
    Write neatly, avoid crossing out or overwriting anything.

  3. Agree and sign
    Try to agree with the other driver on what happened — both must sign the form.

  4. No agreement? No problem
    Fill in your side of the form and note the other driver’s details. Call the police if needed.

  5. Injuries? Always call 112
    If someone is hurt, contact emergency services immediately.

  6. Disagree with their version?
    Write your side in box 14 (“My Observations”).

  7. Get witness details
    If anyone saw the accident, collect their names and contact info.

  8. Don’t leave blanks
    Unfilled sections can be used against you later. Include all important info right away.

  9. Double-check their info
    Verify the license plate and insurance details using their green card.

  10. Check the back
    The back of the form has more instructions — follow them for full coverage.
